Introduction

Embarking on the journey of an autism assessment can feel overwhelming, for both families and schools. At Momenta Connect, there’s a deeply collaborative, supportive process designed to help young people reconnect with education through tailored guidance, therapy, and coaching. We know that understanding what to expect can bring clarity and peace of mind for all involved.

Here’s a step by step run through of our assessment process from start to finish:

1. Initial referral and swift enrollments

When concerns arise, whether due to school difficulties, emotional challenges, or missed learning opportunities, a referral to Momenta  360 can be swift. Schools can typically initiate an autism assessment within 48 hours. This speedy turnaround sets the tone; timely support is a priority, not a promise.

2. Comprehensive, multi-domain assessment

Momenta  360 starts with a structured, early-stage evaluation that looks beyond academics:

Academic needs: identifying areas of learning that may benefit from specialist attention.

Emotional and social needs: clarifying well-being and engagement factors.

Practical needs: understanding routines, preferences, and any barriers to maintaining attendance or focus.
This holistic assessment ensures subsequent support is intelligent, tailored, and empathetic.

3. Personalised learning and reintegration plan

Following the assessment, Momenta Connect co-develops a bespoke plan combining three pillars:

Live, small group online lessons in core subjects.

Therapeutic support, offering one to one care tailored to emotional and developmental needs.

Reintegration coaching, guiding the student back into school life with support and structure.

This multi-faceted approach helps rebuild confidence, learning habits, and routine gradually and sustainably.

4. Daily learning, ongoing coaching and regular check-ins

Instead of a one-off intervention, Momenta  360 offers daily engagement, aligning with a structured school timetable via online groups. Students also benefit from regular coaching, including motivational support, attendance tracking, and emotional check‑ins, to ensure progress and resilience stay front and centre.

5. Gradual reintegration back into school

Reintegration is never rushed; through close collaboration with families and schools, the reintegration coach ensures each step back into the physical classroom is thoughtful and effective. Removing barriers, building trust, and maintaining engagement are core to this phase.

6. Ongoing communication and support

Momenta Connect doesn’t disappear after reintegration. Instead, they provide continuous updates to both school staff and families, ensuring the right level of support remains in place and adjustments are swift if new needs arise.

Empowering the journey forward

An autism spectrum assessment with Momenta Connect isn’t just about diagnosing, it’s about empowering. From a fast, compassionate start to a deeply personalised plan and sustained support, the process is designed to foster progress and confidence. If your child is struggling to engage academically, emotionally, or socially; Momenta Connect offers a warm, smart, and effective way forward.
